A new systematic review just shed light on how very-low-calorie ketogenic diets (VLCKD) reshape the gut microbiota in people with obesity.

VLCKD boosts gut diversity
People on this diet had a wider variety of gut bacteriaโgenerally a sign of better gut health.
It increases Akkermansia muciniphila
This helpful bacterium supports metabolism and strengthens the gut lining.
But it lowers Bifidobacterium
Thatโs a common probiotic, so the diet may need balancing with supplements.
Microbial balance shifts
The ratio of two major bacterial groups (Firmicutes and Bacteroidetes) changedโsuggesting a reorganisation of the gut ecosystem, not necessarily a bad thing.
The most notable effects were observed in adults over 30 years old and those with moderate obesity (BMI 30โ35).

๐ง Ketogenic diet and the brain: New evidence on neurological health

Can what we eat really change how our brain works? A new review says yes.
It explores how the ketogenic diet influences key neurological conditions: from epilepsy to Alzheimerโs, migraine, Parkinson, depression and multiple sclerosis.
The findings are really promising:
In ๐บ๐ถ๐ด๐ฟ๐ฎ๐ถ๐ป๐ฒ, the ketogenic diet ๐ฟ๐ฒ๐ฑ๐๐ฐ๐ฒ๐ฑ ๐ฎ๐๐๐ฎ๐ฐ๐ธ ๐ณ๐ฟ๐ฒ๐พ๐๐ฒ๐ป๐ฐ๐ ๐ฎ๐ป๐ฑ ๐บ๐ฒ๐ฑ๐ถ๐ฐ๐ฎ๐๐ถ๐ผ๐ป use more effectively than low-carb diets.
In ๐๐น๐๐ต๐ฒ๐ถ๐บ๐ฒ๐ฟโ๐, higher blood ketone levels correlated with ๐ถ๐บ๐ฝ๐ฟ๐ผ๐๐ฒ๐ฑ ๐ฐ๐ผ๐ด๐ป๐ถ๐๐ถ๐๐ฒ ๐ฝ๐ฒ๐ฟ๐ณ๐ผ๐ฟ๐บ๐ฎ๐ป๐ฐ๐ฒ.

In ๐ฑ๐ฟ๐๐ด-๐ฟ๐ฒ๐๐ถ๐๐๐ฎ๐ป๐ ๐ฒ๐ฝ๐ถ๐น๐ฒ๐ฝ๐๐, seizure frequency dropped by up to 50% under ketogenic protocols enriched with medium chain triglycerides (MCTs).
In ๐บ๐๐น๐๐ถ๐ฝ๐น๐ฒ ๐๐ฐ๐น๐ฒ๐ฟ๐ผ๐๐ถ๐, the ketogenicโMediterranean model boosted brain repair signals and reduced inflammation linked to multiple sclerosis (supported ๐ฟ๐ฒ๐บ๐๐ฒ๐น๐ถ๐ป๐ฎ๐๐ถ๐ผ๐ป, and ๐ฟ๐ฒ๐ฑ๐๐ฐ๐ฒ๐ฑ ๐ป๐ฒ๐๐ฟ๐ผ๐ถ๐ป๐ณ๐น๐ฎ๐บ๐บ๐ฎ๐๐ผ๐ฟ๐ markers).
Beyond its metabolic effects, ketosis appears to restore mitochondrial function, enhance GABAergic signalling, and modulate the gutโbrain axis; all mechanisms that could redefine nutrition-based neurological interventions.

๐งฌ Very low carb diet and fertility: New evidence from Italy

A new study sheds light on the potential role of the ๐ฉ๐ฒ๐ฟ๐-๐๐ผ๐ ๐๐ฎ๐น๐ผ๐ฟ๐ถ๐ฒ ๐๐ฒ๐๐ผ๐ด๐ฒ๐ป๐ถ๐ฐ ๐๐ถ๐ฒ๐ (VLKD) in improving fertility outcomes among women with overweight or obesity preparing for in vitro fertilisation.
This prospective case series included 52 infertile women who followed a structured VLCKD using medical meal replacements and controlled supplementation for 3 months.
THE VLCKD diet consistent of <800 kcal/day, with 20โ30 g/day of carbohydrates and protein intake at 1.2 g/kg of ideal body weight (calculated based on BMI of 23).
After 3 months, there was a gradual transition to a Mediterranean-style diet.
๐๐ฒ๐ ๐ณ๐ถ๐ป๐ฑ๐ถ๐ป๐ด๐:
A staggering 58% total pregnancy rate (natural + ART) was reported.
68% achieved >10% weight loss while preserving muscle.
Significant improvements in lipid, blood sugar and liver enzyme markers (e.g. glucose, insulin, HOMA-IR, lipid profile).
No serious side effect reported.
The VLCKD appeared to be an ๐ฒ๐ณ๐ณ๐ฒ๐ฐ๐๐ถ๐๐ฒ, ๐๐ฎ๐ณ๐ฒ, ๐ฎ๐ป๐ฑ ๐๐ฒ๐น๐น-๐๐ผ๐น๐ฒ๐ฟ๐ฎ๐๐ฒ๐ฑ nutritional strategy to optimize metabolic health and possibly ๐ฒ๐ป๐ต๐ฎ๐ป๐ฐ๐ฒ ๐ฟ๐ฒ๐ฝ๐ฟ๐ผ๐ฑ๐๐ฐ๐๐ถ๐๐ฒ ๐ผ๐๐๐ฐ๐ผ๐บ๐ฒ๐ in women awaiting ART.

A well-designed ketogenic protocol can turn the pre-IVF waiting period into a real therapeutic window, improving both metabolic and reproductive potential.
